Water Testing Request

 

INSTRUCTIONS

EPA recommends FIRST-DRAW SAMPLES from all fixtures used
for consumption. Prioritize high-risk fixtures.

NOTE:

FLUSH DRAW

Identify potential heavy metals BEHIND the fixture. Use this method to test the water further upstream from the fixture, including the source and any distribution plumbing (city or well). Different situations require different amounts of flush time, depending on distance of the faucet from the source.

OR

FIRST DRAW

Identify potential heavy metals IN the fixture. First-draw samples are intended to test the water as it has been sitting in your home’s pipes, typically overnight.

Before taking the FLUSH sample, flush the pipeline in your structure completely. Run water for at least 3-5 minutes, or until the temperature is constant.

OR

Do not run water for at least 8 hours. Collect water in the supplied bottle immediately as it first comes out of the faucet. Fill the bottle completely to the top lip.

NOTE:

The bottle must be filled completely for a successful analysis. Make sure the cap is tight so that water does not leak during shipping.

Fill out label on the sample bottle. Place bottle in provided plastic bag and check for leakage.

Complete the Chain of Custody form. Date and sign.
